CZECH REPUBLIC, SLOVAKIA - data for the Czech Republic and Slovakia, where available, are shown separately under the appropriate country name.

DEMOCRATIC REPUBLIC OF THE CONGO - formerly Zaire.

INDONESIA includes East Timor.

JAPAN includes Okinawa.

MALAYSIA comprises Peninsular Malaysia, Sabah and Sarawak.
